Head covering is considered mark of respect. It also helps maintain body hygiene, head lies, dandruff.......protect self as well as prevents other from getting what you may have.
Foot ware was left out side to prevent tracking dirt, most Hindu house hold and most people in Western house hold change there shoes at threshold before entering their house.
I have slippers for house. Shoes for yard work, few shoes for for dressed occasion, for daily outing, for work, for walking, for hiking, for swimming flippers or beach ware.........
I do not go without shoes. But I take it out, if occasion demands, like at airport or at a temple or at a mosques, even certain area of the church or when I visit a house hold that has a practise of removing the shoes.

Hindus.
It is not a custom to cover the head in SOUTH INDIA when you enter a Temple.
Maybe In Noth India they covered the head to show respect.
Shoes are removed as they bring dirt in the Temple.

Temple is a Place where God Family is installed and Prayed. Neatness is Must.
Bare foot to avoid Dust inside and Covered Head is for safety to Hair which easilly catches Fire due to flame Light inside Temple, since Devotion is as such, and proper care is must.

Because that is a good practice. Iam not too sure about head covering though.
But you could be in any state, important thing is your heart, which should be free of any worldly demands. You should ask only for grace, for a saint to take you to the spiritual world.

in olden days footwears are the symbol of richness. for seeking blessings one should surrender himself to god or guru. surrendering means get rid of everything. so u leave u r footware outside before enternig a holy place.
covering head is not that much significant. infact in some temples/ gurukul's one is not allowed if he is wearing any hat/cap or any covring on head.
