I know a Canadian Citzen that owns a home in the USA and live there all year round. She works under the table, bought a car there and uses a canadian residence for the insurrance clam. She only comes to Canada for medical reasons. Shes in Canada only for a few days. Is this Legal?

Nope, not legal on either side of the border. She is an illegal immigrant in the U.S. if she has no status there, and she is illegally obtaining health care in Canada, even though she is a citizen, by not living there 6 months a year.

No. If she is from Ontario here is what it says to be eligible for OHIP:

"you make your permanent and principal home in Ontario
and you are in Ontario for at least 153 days of the first 183 days immediately following the date you establish residency in Ontario (you cannot be absent for more than 30 days during the first 6 months of residency)
and you are in Ontario for at least 153 days in any 12-month period"

I am assuming all other provinces have the same regulation.
