is it:"it was started to be applied" or " it started to be applied" or " it was started to apply"??

"It started to be applied" is the only correct form of those three. I wouldn't call it a true passive though, for that I'd say "its application was started". "It started to be applied" does sound better though.

"it is started to be applied" but not " WAS started" because Present Simple Tense
or "applying is started"
