We had this same thing with our son. We took away his daytime naps and he doesn't get milk at night, I think it upset his tummy. I played more games with him and got him really tired out. Then when it was dinner time he got to choose what he had (so that he would eat more and get full). When bed time came around he was tired and even though he still fights it, the fighting doesn't last long. Also when he didn't like his bed we put up a small tent in his room and put in extra blankets and sleeping bags for softness. He loves his sleeping bags now and hardly ever gets up in the middle of the night.

I presume you have tried all the wind down things at night, warm baths, reading, and cuddles?
At his age he should probably have given up his bottle, and if hes using it as an aid to get back to sleep then he will need to learn to sleep without it, this wont be an over night process but one I would incourrage doing. As for getting up, take him right back to bed, with no fuss, tuck him in say good night and leave, keep doing it till he understands that bed is where he needs to be. this may take a week to get in the swing of it, but it will pay off...when you send him back to bed dont make a fuss or carry him, just hold his hand, dont talk to him apart from saying goodnight, hes turned it into a game even if he doesnt realise it and your playing the game, just remember your the parent and you rule the house...not the other way around.
Sorry if this sounds blunt...but hopefully it helps

Stop ALL day time nap and NO bottle at night in the bed.
When you first mentioned 'infant' - I thought you meant a nearly newborn. A two year old is a 'toddler' - NOT INFANT!!!

It my be due to the environments! why don`t you try by changing the environments like place & direction of sleeping?
And secondly try to feed solid or semi solid foods instead of feeding milk...if these two is not given the solution...
Then the third option is better to cosult a children`s specialist.

yes,my childrens doctors have always told me that they get like this at night when they are laying down sleeping because the mucus all goes to the back of the throat and causes the tickling,,so he suggested that i elevate her head when she slept,,,so i put a few firm pillows under her fitted sheet on her crib so that it was safe that she wouldn't suffocate on them but so it would elevate her head,,it kept it all from settling in the back of her throat and she was able to sleep relatively well....
