You can include the cost of a guide dog as a medical expense on Schedule A. Here's a quote from IRS Pub 502:

Guide Dog or Other Animal

You can include in medical expenses the costs of buying, training, and maintaining a guide dog or other animal to assist a visually-impaired or hearing-impaired person, or a person with other physical disabilities.

Based upon this, all costs including purchase, training, food, vet bills, etc. would qualify as medical expenses.
