From your employer. They ARE allowed to make a reasonable charge although most do not. If they do have a fee for it, there's no way to get it for free unless they decide to waive the fee.

The employer may be able to get a copy to you, but if you used a tax preparation firm, they may have one also. The IRS does have a record of the information on there, because it's transmitted to them along with the W-3 that the employer has to file, but getting a copy or getting the information from the IRS may well be like pulling teeth.

Check with your employer(s) for the year 2005. They should be able to furnish it to you.
