If you kept a good doctors that you saw records list they can go way back. Sometimes records do get destroyed or lost over the year's they are your records, I learned this the hard way. After a x-ray or CT get it & keep it. In your case you just need to know the Dr. name & sign the release of Information form from HIPPA and your doctor will have all your records that you request. Good luck.

Where is the Doctor going to look for it? You provide the medical history if you are going to a new Doctor. Unless you provide him with the Dr.s or Hospitals where you were treated there is no way. Also, you must sign a form that allows them to request your medical history from specific places. So, calm down your medical history is protected from prying eyes
