Ring around the rosy was in reference to the bubonic plague-it referred to the "ring "around the "buboes" that later became boils. Pockets full of posies were in reference to the posies or other sweet smelling flowers or herbs the wealthy wore in their shirts to wards off the "foul vapours" that were believed to spread the disease. The bubonic plague was carried by fleas that lived on rats. China still has outbreaks.

Actually, it is associated with the Black Plague. The "pocket full of posies" part refers to stuffing one's pockets full of flowers to mask the smell of death.
God, that's morbid...but it's true.
