I'm going to play a game in English. I'm trying to translate the lines into English. Could anyone help me please?

"The word piano was from italy. Is it right or wrong?"
Should I say "Is that right? " or "Is it right?"
If you find any mistakes,please let me know.

I think what you have is ok, but I think it would be better to say,

"The word piano is Italian. Is that right or wrong?"

Although that is still sort of awkward. Another way you could say "right or wrong" is, " Is that true or false?" True or false is more common in this type of situation.

A better way of phrasing the question would be: The word "piano" is Italian in origin. Is this correct?
Italy is the country, Italian is the language. The word does not 'come from Italy', it comes from the Italian language or from Italian. It's like saying "the word fromage is from France" as if it was immigrating. The word "fromage" is French. The word "piano" is Italian.
In other words, Italy is a proper noun place name. Italian is the adjective which means of or pertaining to a characteristic of Italy, it's people or culture.

An English speaker, speaking in the context of a game, would probably say:
"The word 'Piano' comes from Italian. True or False?"
Then the answer would be either "True" or "False".
*or*
"The word 'Piano' comes from Italian. Is this correct?" (you could also use 'right', but it's more informal)
Then you could say "Yes, it is correct" or "No, it is not".

You might want to say, "The word piano is Italian (or has Italian roots depending on the history of the word - the unabridged Websters can help with that.) Also, if you are posing it as a question, you might want to rephrase it entirely to read "Is it correct that the word piano is Italian/has Italian roots?" Hope this helps!
