You need to make sure that you get to the airport at least hour and 15 min before your next flight. So let's say you only have 3 hours.

I used to work at the airport in PR, and you can get out of it easily and there are taxis everywhere. You will be in Old San Juan in 30 min (the most) after you exit the gate. It is only about 15 min away but count on some traffic, just in case. You have 2 1/2 hrs left.

Count with another 30 min to find a cab and get to the airport. So you will only have 2 hours in Old San Juan.

It is totally worth it! But be careful to walk around too much and be late for your flight. You will only have time to walk to the rest and back to the cab!

If that is too close or you then I would suggest you stay in Isla Verde area which is next to the airport. I would go to El San Juan Hotel that has a gorgeous lobby and eat at one of their restaurants, that are very good. You can walk on the beach at the back of the hotel and you will only be 5-10 min away from the airport. Literally next door.
This link will take you to the hotel's website and resturants information. http://www.thesanjuanhotel.com/activities_and_amenities/dining.cfm

Go to Metropol Isla Verde Restaurant, the food is great, they make great mojitos and the prices (for PR) are really good. Keep in mind there may be long lines at the airport security, so try to get bck at least 1 1/2 hours before the flight.

Depends on where you are. If you in the metropolitan area take one day to walk in the Old San Juan where you can find good eating and drinking in a beautiful historic colonial city. Don't miss El Morro Castle. Take another day to visit Plaza las Americas in Hato Rey the biggest shopping center in the Caribbean with lots of places to eat and of course good shopping.
