Low self esteem. People with low self esteem always perform at sub-standard levels.
When questioned about their work they will get defensive and have excuses which can be legitimate and in no means can you call it lying as you are crossing over into gray areas with labor laws and yes, even sexual harassment falls under this category as well.
Since you are this employee's co-worker, instead of being judgmental, what you should do is find a positive aspect of their work ethics that you like and embellish on that which will help raise their self esteem. Instead of stating "you did this wrong", state, "I see how you did this, here is a suggestion on how you could do it differently in the future". Give th employee some credit and a chance. There is a reason why that person is still employed with your company.
On the other hand if there are other issues involved besides sub-standard work, such as tardiness, excessive absenteeism, personal phone calls etc., then that is something the supervisor/manger needs to discuss with them.

Pathological Liar. I had a room mate in college like this, I could never tell what was the truth and what wasn't. I didn't realize what a big deai it really was until I met her mother and found out how many lies she really had been telling. But your situation may be that she is just lazy and prefers to push the blame on someone else instead of fessing up...in denial of her own inability to the work and would rather pass the blame onto someone else.

Pathological liars do it to serve some need inside
Habitual liars lie because it's a force of habit
They're pretty much the same, they just sound different

I wouldn't call lying under those circumstances "pathological." It's just more comfortable and convenient for her to lie than to tell the truth. She's just lacking in integrity; not unlike most people you'll encounter on the job. The word is "slacker."

Pathological liar. My ex husband used to be one. Very good at it he was too. He lied about his past and even to the little things like what he bought at the grocery store. Very strange because he didn't need to lie. He just was compelled to lie.
