The word "Rabbi" as applied to a Jewish leader was first used for Ramban Gamaliel around the middle of the first century.

The leadership position existed since Mt. Sinai, with Moses being the first, and he picked judges from among the people to also fill the role, as per the suggestion of his father-in-law Jethro.

Now comes the weird part - The last rabbi with true rabbinic authority dies in the 15th century. His name was Yosef Caro, and he wrote a Jewish book called Shulkhan Arukh.

Today the people that are called Rabbi, are only called it as a politeness. They are not real Rabbis, but they are supposed to be conversent in the section of Yosef Caro's book called Yoreah Deah at the very least.
