I think this simply makes them feel as though they can offer an opinion where none has been asked for.
"I don't want to intrude but", "I don't pry in my children's relationships (referreing to adults) but"
The "no offence but" is offensive.
I found that it is key to interrupt them and say "good, don't". These are all passive-aggressive intros.

I use 'no offense' or 'sorry, but' when I answer a question but I know the asker is probably not going to like the answer... sometimes the answer just isn't pleasant. An example, take declawing -living in a country where it is illegal, I'm absolutely convinced it's cruel, as well as unnecessary (people here in Europe have loads of cats, loads of nice furniture; if we can train our cats why shouldn't Americans be able to???). If someone asks a question about declawing but makes it pretty obvious they hope to hear it's all cool and fine and convenient, then sometimes I'll use phrases like 'no offense' or 'sorry, but'. It's a way of showing I don't argue with them just out of spite but rather because I honestly believe in my point of view, I don't want to shut up and say nothing but neither do I mean to be offensive, it's just my opinion but it is a strong one and that's why I try to take a little of the heat off with these phrases. It is as when we say "good morning" and we wish they were dead! As when we ask "how are you?" and we can't care less!
I agree with you, people believe that this "no offence" gives them a perfect excuse for being rude and insulting.

I've never quite figured that one out myself; they seem to think it takes away the sting to say "no offense" - which is about the dumbest thing I ever heard!

yeah i know! when someone goes 'no offence BUT' you just know that something offensive is gonna be said! or when somone goes 'nothing personal BUT...' or 'i'm not trying to be funny with you BUT...'
