It is very tempting when you are pressed for time or having something more amusing to do to put off a mundane sewing job, such as mending a hem. If you were to do the mundane job when it first became evident, then you would only have to do a few stitches and everything would be well. If, on the other hand, you delay getting on with the job, then the hem will inevitably catch and you will be left with a major sewing job on your hands which will take much longer than those few timely stitches would have done.
I have experienced the truth of this very often and now make a point of dealing with things of this kind immediately! It's the same for so many household tasks -- delay doing them and they become major chores!

If a garment shows signs of a tear or rip, a single stitch (not 'stich') put in at once will be enough to mend it. But if the tear is left it will get bigger, and at least nine stitches will be needed for the repair. Hence the proverbial use of the expression: Immediate action to put something right will save much trouble later.

You take action in time to stop some damage, and get the repair done quickly without postponing, sothat the damage will not become worse or the repair will not become larger inscope! if you don't stitch up when you see the first tear, the tear will get worse, bigger, you will later need many more stitches to repair the torn cloth!

literally speaking: if your clothes get torned and you do not stitch them on time you might regret it afterwards.
as for what the proverb refers to is that it is better to deal with a problem when it is small rather than waiting for it become the most horrible nightmare of your life. the proverb also emphasises on the importance of doing things on the right time so that you don't have to regret

If you have a accidental tear in say a shirt if you do not stitch it straight away it will get bigger and require nine stiches or more instead of say one at the start.
