Go to his bed room if you live in the same town and see what he like. or if you do not live and the same town ask him what he would like or just give him a $100 and tell him you love him. Money always put a smile on a persons face.

Your favorite inspirational book or a journal. Find a baby picture of the two of you have it changed to black and white and write something really neat around the matt of the frame.
If all else doesn't work remind him how much of a pain he was when he was little in a card and have two movie tickets and go see a movie together, buy the popcorn (just make sure you toss some at him). After all he is still your little brother. :]
