You know what, sometimes that is good enough. Generally when an employer wants a sick note, they just want proof that you have even gone to the doctor. If he asks for more informaton on the note, go back later.
If you are caught forging the note, not only is it illegal, but you can be fired on the spot from your job.

Just ring the Dr's up and say work need a more descriptive note. If you give the same note back to your employer with your forged description you could also lose your job and a nice court appearance!
To the person below I haven't mentioned jail.....

Tell your employer it is none of his bloody business beyond what the doctor has written. The doctor has written a note, its legal, anything else your employer wants to know is pure invasion of privacy and not a legal requirement.
If I am ill my doctor always asks me what i want on a sick not. If it is deeply personal/gynacological he is just as happy to write "gastro" as anything else, and my boss can shove it if he wants anymore info.

Usually the reason is enough, however not sure what your reason is , plus I WOULD NOT add anything to the note, simply call in and explain to the receptionist and she may nip and and your doctor will have it done in seconds. Your boss sounds a right funny duddy.

trace each letter you need with the letters he's already used. You would need to be quite skilled a forgery to get away with this though...................
