Is that how it happens, if so, how long would the tampon have to be there?...

If not, how is it caused(what are other ways for it to happen)?

I'm refering to the FLESH EATING DISEASE not TSS. I should have made that more obvious.

Toxic Shock Syndrome is caused by a particular bacterial toxin [usually a strain of Staphylococcus; the tampon dries out your vagina and makes it easier for it to enter into your bloodstream. You can also get TSS through breaks in the skin or through your throat. TSLS [short for Toxic Shock Like Syndrome] is almost the same thing, but happens due to a strain of Streptococcus. If Streptococcus attacks your skin, muscle, or connective tissue it could cause necrotizing fasciitis [though this can also be caused by other varieties of bacteria as well].

Basically, there is a very tenuous link between the two, but it's unlikely especially if the TSLS is treated. If you're especially paranoid, you might want to try using a menstrual cup [such as The Keeper or DivaCup] instead of tampons; there's no observed link between cups and TSS because they don't absorb moisture.
