When they say "Attach your W-2" do they mean with a staple? paper clip? or just include it in the envelope?

Yes, with a staple. There are 'marks' at the edge of that sheet where you would attach by stapeling the two pieces together.So the small sheet(W-2 form) is in the middle of the bigger form. When you fold up to put in the envelope, it stays in the middle.

Paper clip, then include in the envelope.

Staple it on the left hand center of the first page.
