For Sunnis shahadah has 2 parts but for Shiats have the same parts plus one other part.
Sunnis talk about God and Mohammad (pbuh) in their shahadah.
Shiats talk about God, Mohammad (pbuh), and Ali (alaih salam) in their shahadah.
A Shiat shahadah is:
part 1 --- Ash-hado an laa elaaha ella Allah
part 2 --- Ash-hado anna Mohammad-an rasoul Allah
part 3 --- Ash-hado anna Ali-on wali Allah

The Sunni Shahadah is :
"There is no god but God and Muħammad is his messenger"
The Shia Shahadah is :
"There is no god but Alláh, Muhammad is the Messenger of Alláh, Alà is the Friend of Alláh. The Successor of the Messenger of Alláh And his first Caliph."
Shia muslims have for Leaders Infallibles Imams. First being Ali Ibn Talib and last Imam Mahdi. They believe that the sunnah are better known by the Ahlebayt (family of the prophet, Fatima, Ali, Hassan, Hussayn etc) and whoever follows the Ahlebayt are as safe as those who embark in the ark of Noah. There are numerous Hadiths (Traditions) which shows clearly that it was the prophet's will that Imam Ali become the first caliphate. Though Bark with Umar and someone else plotted differently and achieved their plot and Bakr being the first caliphate of Islam, Shia muslims know who are the righteous successors of Muhammad and spiritual leaders of Islam and reject the evils in disguise.

Bloody Dagger, you got it wrong...It's the other way around. It's the Sunni who murdered Ali and declared they would appoint their own "leader"....The Shiite respected Mohammed and what he had said concerning Ali being his successor....
As far as Declaration of Faith goes, I suppose it is the same..you just Declare that there is only One God and that Prophet Mohammed was the Last Prophet to be sent from God...

The word "shahadah" comes from the same Arabic root as "shahid," which refers to a suicide murderer.
I don't think that your translation "declaration of faith" is intended to be literal.
