It's just a speech crutch, if you point it out they may change it. Recently the radio DJ's here whipped together a bunch of these that a couple of them use all the time, one is "actually" and I forget the other, they just took the clips from several months and put them together, it was odd. Nothing wrong with it, if you think she uses it too much mention it to her that you've never heard it before and wonder where it's from etc(this is better than "you sure say that weird thing way too much weirdo").

Like you said, it must be a habit.
She probably isn't around other people enough,
or they would set her straight, and help her with this problem.
I'd point it out to her by asking Each time, "On what?"
and see what she says!!
It would increase her awareness that it doesn't make sense.
Then she may talk about it with you.and you could tell her what you notice. You are kind to help her. It's her problem, and not yours, except for kindly putting up with it.

there is a syndrome named "Tourettes Syndrome" which varies in degree from mild,almost inobvious, to a person who makes loud noises and has no control over the interruptions they cause. if you search on-line i think there will be a lot of information. Tourettes also causes some people to make clicking noises, some people make sniffing , grunting noises, and other use profanities.
after you read about tourettes my guess is you will recognize people who have this syndrome.

While I am sure there is some medical/psychological reason for most people doing such things, I too have a co-worker like that, but I am 110% sure the reason she does it is pure STUPIDITY.
