You file it on state income tax return form. In Illinois it's IL-1040 or IL-1040EZ. Some states even offer easy on line filling.
Go to your local library and pick up the forms and instructions or go to your state's treasurer's site and download forms there or learn of other options, such as on line or phone filling.

when you grab the package for federal there was a part for the state in there also. If you did it on line it tells you to go to the state . Call IRS to get that form
