aparently SAMI has not been to a doctor in years. We currently use otoscopes, which use lenses to magnify and focus the external auditory canal and the tympanic membrane. The light shines around the tube through which the physician is viewing, and comes from a light bulb, not a reflective plate worn by the physician.
So, physics is applied by the optics used in the lenses as well as the electrical circuitry used for the light source.

The doctor generally an ENT specialist wears a concave mirror on his right eye with a small aperture in the center for doctor`s eye. A flood light focuses light on the concave mirror and the beam of light is reflected by the concave mirror to focus on the external ear,auditory meatus and typmanic membrane . Through the small aperture the doctor examines these brightly illuminated parts.
