I usually try to charge a basic sq. footage price and then add extras if I need to make more on the job. Tile about $2.25,
Hardibacker or cement board, $0.65. Sometimes if the job is extremely small, I will make very little $ with this base price so it's necessary to add costs for floor prep, removing toilet or appliances/furniture, removing baseboard etc. I've found that this is more effective than raising the per ft. price and seeming too expensive to the customer.
I work in Ohio and I think my prices represent the low average price in most of rural America.

A floor tiler will want to know how much preparation is required. What the access is like. Which materials are to be included in the price and so on. Also some tiles are more difficult to lay than others. Apart from the area of tiling, there are tiles that need to be cut and tiles that are used for borders, and tiles laid in an extraordinary pattern, and floors that are difficult to get falls in because of the design... There are as many variables as there are bathrooms.

Like any job, by any trade, a tiler would be a complete idiot if it hadn't tried to calculate the cost of the job from the bottom up.. the cost of materials plus labour, and add a margin for error, waste, overheads and profit... before giving its customer a lump sum price.

Obviously some tradesmen will provide a per sqm price based on their expereince, that is, based on the last similar project they performed, and whether they made a loss or a profit. This is because everyone wants a price and not everyone proceeds... and tradesmen can get pretty sick of pricing jobs that don't proceed... so a sqm rate is convenient but a bit risky... less risky with floor and wall tiling than for say the carpentry contract for building a house.

How do they figure the amount per sqm? Guesswork based on experience. What is the going rate? depends on whether materials are in or out of the rate, or partly supplied by owner.

Too many variables to simply quote a flat rate price. If you are talking about a square room with few corners, average tiles and a straight forward patter, then a sq ft price can apply. Usually about $2 a sq foot for a simple job.

Including the pad or whatever it is they put under the ceramic tile first..here in colorado the going rate is 3.00 - 3.65 per sq ft..or thereabouts

probably depends on where u live and if they include that layer underneath the tiles
