Had an ex- do this for me: basically it's a day of fun...spend the afternoon outdoors (for us it was the beach), then in the evening, rent a limo (so you can drink...or skip that if it's too much $$), take him to his favorite restaurant, a movie he wants to see, and a club he wants to go to (comedy or dancing...she took me dancing).
It involves nudity, but I had the night capped off with some wild freaky fun, then while we were chillling and watching TV, she breaks out with a six pack of my favorite beer. That did it! I almost married that girl!
Basically show him that you know him in one solid day. Then don't get mad when he can't reciprocate quite as well on your birthday (we try, but we're not genetically engineered to remember that kind of stuff)...

U could make him dinner (if ur a good cook), and then go back to his place and play romantic music and make out. I am not saying have sex, but just see where the making out goes. If he wants to put his hand up ur bra, why stop him?? I mean, come on. It's not like it doesn't feel good. :)
