Hi everyone!

First thing, I am not happy in this job but I need to do this before I resign.

I need to create some sort of a module for Monthly performance Evaluation for the inbound agents. Everything is all set up except for the suggestions part.

"This is the final activity. The agents will be given a piece of paper for them to write their suggestions for the department they belong to. Any comment or suggestion will be considered with utmost confidentiality and if proven necessary actions will be carried for the certain suggestion/comment."

my plan is to divide the form to two parts like : Department , Administration.

Can I solicit from you guys questions for this?

The questions should be answerable with yes/no or with 1-5 (5 being the highest)

You can't really solicit suggestions for improvement by asking them to rate things on a 1 - 5 scale. That scale is more for employee satisfaction with existing programs, like:

-How satisfied are you with Company A's benefit programs?
-Do you feel that you received the appropriate training for your job?
-Does your manager give you frequent feedbacfk about your job performance?

To ask for suggestions, try a "fill in the blank" concept by asking:

-what one thing should the company continue doing?
-what one thing should the company stop doing?
-what one thing should the company do differently?
