If you are carrying a balance on the card then yes, you'll have to pay. Even if the balance is zero, you may have annual fees, etc that have to be paid.

WHy would you get a bill if you haven't used the card?

Bill for things purchaced or application and processing fees.

depends on the small print in the contract. some make you pay a fee for the card regardless if you use it or not. it just depends.
