Complete information including forms, photo requirements and where to pay are available at this site: http://travel.state.gov/passport/passport_1738.html.

There are travel agents that for a fee with assist you in the process but the lead time is greatly increased due to staffing shortages at the federal facility. Plan to receive your passport up to 30+ days from the date of filing. Good Luck!
