If you are referring to the traditional Japanese loin cloth, that would be called "fundoshi". Otherwise, whenever I have gone to bathhouses in Japan, guys would have an extra little hand towel that they would lay across their private area. I have even been to a few places where the men walked around and entered the bath freely. This was uncomfortable to me for two reasons. First, since I'm an American, I'm used to seeing everybody all covered up and bathing separately. Second, because I'm an American, all the other Japanese men kept looking and mine and then looking at theirs afterwards to compare. That is actually the main reason I don't like going.

actually, japanese people don't really wear things in the bathhouse. not where i've been, anyway. you can bring a small towel though, but some places don't allow you to actually use it in the bath (you can put it on the side).
