I would go have a nice meal.
It's nice that your boyfriend's mum likes you enough to make an occasion for your birthday. If every girl was that lucky!
At 21.... or should I say approximately between ages 12 and 23 or so everyone feels that they are being watched and judged... believe me when I tell you this.... unless you are a celebrity or inconceivably beautiful or a freak with two noses no one will stare at you...maybe an occasional look to see if you are having a nice time.
If everyone else is eating they'll be too busy with what they are doing.
It's a tough thing to get over what you are worried about but you are restricting yourself from being with certain people by being shy in that manner.
Have a night out with your mates and BF either before or afterwards. Last I heard there's no set rule for celebrating on the day of your birthday.
Hope your enjoy your 21st birthday! Enjoy the attention now so you have some fond memories to look back on.
That's what I would do...spend a couple days celebrating with people who enjoy my company and I, theirs.

admittedly, i'm a little confused with your description of the events...still, i'll try to help.
with regards to the nervous bit, that's really understandable. the thing of it is, you got to try to figure out what makes you nervous around them. often, it's because you aren't that close to them. just think, this might be the opportunity for you to get to know them better. after all, there doesn't seem to be much harm in doing that. at least, when they get to know you and you get to know them, perhaps you'd feel a bit more welcome (maybe even feel like part of the family).
well, honestly, the choice is yours. either way, since it's your special day, it's best if you choose something that you know would definitely make you happy. consider what both options have to offer. you could get to know a family over a meal or re-celebrate the bond you have with your mates. consider that perhaps one option may only happen once or even just a few times in a lifetime while the other could be done some other time. try to be objective about your choice. if you really can't choose, try to come to a compromise between the activities. maybe you could have lunch with the family then go out with your mates for a night at the movies...or even let them accompany you during the meal. it's your call and it's your day. so what matter's most is that you're happy. good luck!

If you are really nervous when you are eating best not to put yourself through this on what should be a carefree day. However, it is a lovely thought from your bf's mum so tread carefully in order not to hurt her. Speak to her on your own and tell her truthfully how appreciative you are but can't help about how nervous you would be. Try and compromise with either something like drinks and nibbles at her house or at the the hotel. Happy 21st!!!

Do all of it! How nice of bf's mum to take you out for a nice posh meal. No-one will be looking at you, as you will all be chatting among yourselves, and concentrating on your own meals. You will probably have a nice toast with champagne, and a cake.
Then have your night out. No-one will mind when it is, as you are getting all these treats and it will be on a day when you can do it.
Enjoy your holiday, and your birthday. Let it stretch out, and have fun!
