Adherence refers to whether the patient follows the doctor's instructions. For example, a patient who stops taking his antibiotics after 3 days even though he was told to take it for ten days is described as not adhereing to therapy i.e. poor adherence by the patient.

Poor adherence to therapy by patients leads to poor treatment outcomes; in the example given above, the patient's infection will come back because it was only partially treated. When it does come back, the bug that caused the infection is stronger than ever.

Example: A doctor puts several patients on a weight loss program where the patients are supposed to eat less and exercise more. This would result in the program not working as well as it could have if the patients actually did what the doctor told them to do.

We used to say the patient is not "compliant" with a treatment regimen. However, the new, more politically-correct term is "adherent." Supposedly "compliant" sounds more like you're trying to force someone to do something, while "adherent" sounds more like a partnership.

Look, if you're going to go to the doctor and pay for the advice, why not follow it? If you're going to do your own thing anyway, don't waste your time/money seeing a doctor.

Seems like you're referring to a course of medical treatment. If that is the case, then what the sentence means is that the treatment is often less effective than it should be because the patient won't stick to it.

The simple answer would be ... poor results/outcome because the patient is not doing what he/she is suppose to be doing. Example, John has asthma but refuses to take his medicine. As a result, John's asthma cannot be kept in check. A patient is commonly called non-compliant when he/she is not doing what the physician has asked.

the treatment plan is being sabotaged by patient non-compliance. it could be purposeful (patient refusing to follow doctor's instructions) or non-purposeful (language barrier preventing patient from understanding instructions, for example).
