If you're worried about the 212 requirement, maybe this isn't the best visa for you. They are near impossible to get and they can take a long time. Instead, look for programs that'll sponsor you for an H-1 for your residency and fellowship. H-1s are valid for 6 years (as opposed to the 5 years for the J) and you can apply for Permanent Residency afterwards if you want. However, they are a lot more expensive than J-1s.

If you are already asking about the possibility of getting a waiver of the 212e two-year residency requirement so that you can stay in the US permanently, you should know that these are hard to get. Also, the program is about training physicians to do better for their own countrymen, not just to get to the US at someone else's expense.
