All pilot ratings, other than sport pilot in the United States, require a valid medical. Of course, if WE take your royal highness' double negative literally, the answer would be different.

No, medical fitness is the prime requirement for pilots.

No, if you are getting paid for it then you have to be medically stable to fly other people. As for the rating...commercial.
