I know if you're convicted of or plead guilty to a felony but are there any more? Are people denied if they have say, unpaid tickets, or don't know the answers to some of the questions on the form?

To the best of my limited knowledge you can only be denied a U.S. passport if you cannot prove your right to U.S. citizenship to the satisfaction of the U.S. Dept. of State.

You can be required to surrender your passport to a court to prevent you fleeing prosecution but you can't be denied a passport due to a criminal record.

Well I know that I left the spaces blank where it asked for my parents' birthdates since I couldn't remember them. And I got my passport just fine.

And there's another reason you could be denied a passport...no money. When I got my first passport I think it cost $35. Now it costs $97.
