Comprehensive coverage, not collision, is what covers glass replacement and repairs. Check your policy to see what your glass deductible is, because it may be more expensive to claim it than to fix it yourself (you may have a 0 deductible, too). If it's just a ding and not a long crack, check your local auto parts store for a windshield chip repair kit.

that would usually fall under comprehensive. Check your deductable and check the cost of your windshield if you bought it yourself. It might be cheaper to replace it yourself. By the way, your insurance premiums will go up a little to offset the claim you file so do some checking.

it will probably depend on what your policy covers. most cases a cracked windshield will be covered no matter how it happens because it can be defined as a safety hazard.

Last time I remember...NO. I asked cuz two of my cars had cracked windows cuz of them semi trucks with the gravel. I think you have to lie if you want to have it covered!
