Just follow your baby's cues! He will let you know how often he wants and needs to nurse.

There is no need to introduce cow's milk as long as your baby is nursing at least 3-4 times per day.

As often as he wants it. My son slowed down on nursing from about 12-13 mo... but then upped his intake to 5 or 6x a day in the past month or so (i think hes having a little growth spurt... hes 14 mo)

As long as your giving your babe good food, fresh fruits and veggies... whole grains, fresh meat... he will know what he needs as far as milk. Follow his lead.

My son is 11 months old and breast feeds on demand.

As often as your baby is hungry. You'll know that she's getting enough if she has five to six wet diapers a day. You can also check to see if she's dehydrated by pinching a bit of skin on the back of her hand (not hard!). If it springs back quickly, she's fine. If it stays pinched up when you let go, she's not getting enough.
