she has been on it for 3 years and has been moving it to different spots.

Please, next time you check your question answer this in an update:
1.- How many cycles has she used the patch?
2.- Does she change places of the patch after each cycle? She should.
3.- Does her skin itch or swell too?
Probably she has an allergy to the patch. Normally she should feel OK using it, no itching, eruptions or nothing.

She is allergic to the patch itself or the medicine in it. Notify her Dr.

Don't keep applying it to the same spot. Move it each time.
