I know there is a 1099 you give your sub-contractors in Jan, but what is the form they fill out when they first start work with you. Does anyone have a link to the exact form?

You should just have a sub-contractor file, with the sub's Name, Address, Social and Phone.

The form they fill out for you is a W-9

W-9 request tax ID # so they can file a 1099 showing your income

You send them a W-9. The put their SSN or EIN on it and return it to you.
