Not really.
If you are registered at a parish then you may ask to be removed from the registration rolls.
However there are Sacramental records that are used as legal documents and cannot be altered. If you have been baptized then it happened even if you wish it had not.
And we believe that certain Sacraments like Baptism and Confirmation put an indelible mark on a person's soul that cannot be removed.

There isn't some centralized list of "baptized Catholics" floating around out there. The church where you were baptized is supposed to keep the official record of your baptism. This isn't a "membership" record but a record of the sacrament. You can't have this record purged. The Catholic Church believes that once you are baptized, you are forever baptized, even if you are excommunicated.
If your intent is to distance yourself from the Catholic Church, just simply stop attending. If you are just trying to get off the mailing list of a particular parish, call the office and ask to be removed from their mailing list. You don't need to give a reason.

There is not a general "list" of who is Catholic, but if you are a parishioner of a church, then that particular church will have a list that you are - you signed up to worship and support that particular parish. But as a Catholic, you can worship at any Catholic church.
I know that the Mormon faith has a list and if you want to leave the church you have to make an appointment with the bishop of your local church(?) and sign something. If you don't do it, the missionaries keep coming to your door to get you to come back to church.
I know because my ex-boyfriend was Mormon and they wouldn't stop knocking on his door.
If you don't want to be a Catholic anymore - just stop going to church - I doubt they'll come and knock on your door.

First of all, the Catholic Church is not a country club. I doubt the Vatican has enough time to keep track of EVERY Catholic walking the face of this earth.
And yes, you can get excommunicated but I don't exactly know how.

Each individual parish is required to keep a list of Communicant Members. A Person is listed in the Parish Register when they are Baptized.
To be taken out of the Parish Register - all one has to do is to write to the parish in question and either:
1) Ask to be removed, or...
2) Ask to have the parish membership transferred.
