It has happened. I saw just recently on the History Channel, a show about scary and unexplainable stuff in the Old West. One was a story of a boy who was buried alive. I do not remember the name of the episode though. The series is called "Wild West Tech."

There was also a mythbusters that dealt with this. I do not remember if they pulled up any actual occurences though. They tested to see how long a person might live if he/she was buried alive.

Also, during Victorian times, there were many stories of bodies being moved and that the insides of the coffins showed scratch marks on the inside of the lid. A lot of funeral homes capitalized on this with death assurance clauses and selling contraptions to allow a person to signal for help. It also fueled a lot of literature. Edgar Allen Poe penned a few stories with the subject of accidental burial. It is said that he had a fear of being buried alive.

Recently, I would doubt it happens too much. Embalming has a way of preventing a lperson from being buried alive.
