She may be sick, such as with a urinary infection, they learn to associate the box with pain so first off, get her vet checked. Cats are good at hiding illness, it's something that is leftover from their wild ancestry in the cat world, a sick cat is a weak cat so they will do everything to not let other animals know they are sick which leaves them vulnerable to attack. A cat can be knocking on heaven's door before you notice there is anything wrong.

My new female cat would do this because she will not set foot in a litter box if there is already something in it (I have 2 other cats, so it was a problem). So we did like you and made sure it was cleaned everyday and actually every couple of hours. But basically it was impossible to keep it absolutely clean every second of the day. So we ended up getting 2 more litter boxes and put all three of them next to eachother. This worked out great, because she has more clean places to go now, becaue for some reason my other 2 cats will only use the original one.... its perfect, like she has her own 2 litter boxes all to herself.
Its funny though, because there will just be one little poop and she wont go near it.
So if you have room, you could give that a try.

There are a bunch of possible reasons:
It could be a health problem: Some female cats are rather susceptible to infections of their ureal tracts. If your cat has such an infection she might simply not be able to make it to the litter box in time. This kind of infections are treatable. A visit with the vet could solve the problem.
If your cat is healthy, there are a lot of behavioral issues, too. Female cats can be much more picky than males are:
How many litter boxes do you have? Some cats just like variety. The usual recommendation is one box per cat and one additional one.
Are you using perfumed litter? Try a few different fillings for the litter boxes, some just might not comply with your cat's idea of proper bathroom inventory.
Sometimes cats return to the places that are already smelly. If the carpet still reeks, your cat might consider it a proper place to do her business. There is carpet cleaning stuff available in pet-shops, which sort of works, but a bit of the smell tends to linger. Removing the carpet might help.
Is your male cat a bully? Or does it happen, whenever visitors come? Or is there something else, that makes her really uncomfortable? Sometimes cats react to stress by peeing astray. (My cat tends to give me a smelly welcome home present, whenever I have been away for a night.)

Put a litter box where they go most often, if they still go on the floor pick up the mess and put it in the litter box so they will know what is the right place for this. Clean the floor with urine-b-gone and spray with a spray that will repel them. These products you can buy at a pet store. Also if you don't mind pepper on the floor, sprinkle it on the floor so when they try to find their spots they will get and unpleasant sniff of pepper. I told my mom to do the same when her cat now 13 years old started to go on the floor instead of the box. She is now using the box again. Eventually move the box little by little to where you want it and they can find it.

Last year my beautiful Manx cat Theodore went out for the evening and never came back. I love cats and the house didn't feel the same without one, so I picked up Lola from a rescue centre. She was very frightened and would pee all over the house. I found Cat Spraying No More� on the internet and the techniques worked almost immediately. I haven't had a problem with Lola since. Amazing!

My female is the same way. She has been with us for 10 years almost and when we had a stray female come in the home she was mortified, she sprayed everything, and I had to get rid of her. And we did have separate little boxes, food dishes, and bedding.
However when a male stray who I had fixed happened upon us, she was fine with him. She still knocked him around a bit, and let him know she was boss, but she did not spray. It very well could be that you might not be able to keep both. My vet said that once they spray it is something that they are detecting in the other animal that they do not like. The bad thing is that they will probably not change.
The for some reason feel offended, and no one knows why.
Like humans you like people and dislike others.

Try another litter box. The rule is, you are suppose to have one extra litter box per number of cats. So you have two cats, you should have 3 litter boxes.

Cats like dogs spray to mark territory there will be nothing that I know off to stop unless you put her in a carrier when you are not home and watch her with spray bottle in hand when you are home. Its easier to stop if caught in the act. Use a black light with house completely dark and kitty cat pee will glow I would clean with a good stain/oder remover at once before starting correction if they smell they will do it again.

Cats sometimes pee outside the box if they have a urinary tract infection, and you should make sure to rule that out because it could become fatal if it goes to the kidneys. Otherwise, she is probably upset about something. Give her some extra attention, and see if that helps.

Some cats will not pee in the same box as the other sex. Sometimes it is a behavior problem. 1 of mine started doing that and it was stones and he couldnt pass them so he had to have surgry.
