Last year I went and had my son and daughters last name changed but didnt know that state of AZ you have to have this sentence on the decree((VITAL RECORDS WILL AMEND BIRTH CERTIFICATE) it was passed but that wasnt on the decree and I would like to change their birth certificates but cant.Someone told me to put in a motion to amend but how would you do that and where would you go to get the paper work because i did eveything myself so I am lost right know.
Thany you for any help you can give to me.

Since I don't anything about the law, I can only suggest to you that you contact the courthouse, a lawyer and ask for legal advice.

SInce there is an existing decree already, I suggest just going to the court and talk to the judge or clerk of court or other court personnel and explain the situation to them. Normally, you would have to file a motion to amend a court order or decree, but since the issue is a non-litigious matter I believe the judge can just change the decree motu proprio (of his own accord). Can't hurt to try it that way and it will save you having to hire a lawyer.

The decree has to be amended in court, it may be as easy as getting with a court clerk and scheduling it since it was just a name change. Or you may have to get a lawyer, depends ont eh state.
Keep all the paperwork in a lock box as this will be required as your kids grow up...
