I would go with:

Hopefully, they will fill in the names of all attending, and mark a number of each meal they choose:

Otherwise, if they only fill in "Mr John Doe and Family Will Attend" and put an "X" next to the meal choice, you will have to call and find out how many of that meal they want. You could leave off the "Will Attend" because if you have meal choices and they have selected one, they are obviously attending. But you would still need a spot for "Will not attend" Something like:
M__________________
___Beef ___Chicken
___Fish ___Children's
____Will not attend
*Please fill in quantity of
desired meal.

From a caterer's perspective, you have too many choices, your guests are going to pick what appears the yummiest regardless of the card they sent you several weeks ago. Better to have a buffet with all those choices. Set all your tables for dinner service to look elegant. Have salads in place when guests sit down and call them up by table numbers to the buffet. They pick up thier plates and go to the buffet (all the other stuff, silverware, napkins, glasses, are all at thier seat already) This also saves food too. They get what they want in the quantities they want. The guests, caterer and you will be happier, and you will save money and be less likely to run out of food.
People just don't use the RSVP cards effectively anymore.

I have to admit that in all my years I have never ever seen a menu choice on a wedding rsvp card..... after all its not an inflight meal. Choose your menu, don't inform the guests of it and just ask them on the rsvp card to let you know if they have any special dietary requirements, that way you cater to a few people who have particular needs, and the rest of the crowd get exactly the same as each other - making it far easier for everyone involved!

I did it that way. On the rsvp card I also had the dinner selections listed and requested that each person attending would initial their choice. It worked well. Good luck

On the reply card include a section on meal choice. just like the blank where they fill out how many are attending they fill in how many want which dinner choice.

First there is the invitation ....and then there is another card for the reply and the choice of meal if they choose to come. Then enclose a small envelope for that reply card, pre-stamped.

or go to a place like Pip or Kinkos who does cards like that and ask them in person, they will show you how it's done.

I have seen real fancy ones with all the names printed on the reply card. But most of the time it's just done by couple and the waiter re-ask you on the wedding date. This allows the cater to get a decent count but people can change their mind if they please. It's really just for the cater to know how much to make.
I am having buffet but we are also printing on the cards- "Please let us know if you require any special like a vegan meal."

Go to www.microsoft.com they have tons of free down-loadable templates which you can look at. The best thing would be to have the choices listed in smaller print at the bottom with a line for a check mark to indicate which is being requested.
