The best Italian restaurant in Little Italy is Il Cortile on Mulberry Street. It's not cheap, but their food is outstanding. You should have reservations.

Note: In terms of both food and price, you will do better outside of Little Italy. Actually, in terms of price and quality and authenticity of food, you will do better outside of Manhattan - Brooklyn and the Bronx have better Italian restaurants costing half of what you pay in Manhattan.
You'll be with your family, so have a great time.

You'll love Paesano's on Mulberry Street. Mention "Opie and Anthony" and get a free glass of wine. The food is good, affordable, and Joey is the real deal ItalianStallion. Sure, there are better places in the city to eat, but if you want the Little Italy treatment of old, goto Paesano's. LaMela is a good time if you want to pay a lot for food you never ordered (they have no menu), and if you are there with a fraternity house- it's LOUD and obnoxious. Another old reliable is Benito's, though I would goto Benito's 2 (one and three always disappoint). Mangia Bevi!!!

If you want to hone-in in that area downtown.. you are looking at Mulberry St. and Carmine St. and Prince St. and Broome St... those intersections. You can walk around, but don't be disappointed if you see more of Chinatown, now that Little Italy is only a small area. In fact, with most of the Italians moving to New Jersey (hello? the "Sopranos" setting, isn't fiction) you'll be lucky to even SEE and meet an Italian-Amer. that's there, simply collecting rents on a family building or working a business still left, from mom/dad. If you really want to go... just walk-around and pick one based on ambiance of the particular restaurant, because there's plenty of seating (summer is their peak season) and they cater to walk-ins, since its a tourist and suburban-tourist market (from NYC's suburbs). Here's a link to a good restaurant.

The best Italian restaurants are not in Little Italy, sad to say. But if he wants a NYC experience, walk around Little Italy and then go to Forlini's. It has great atmosphere, food, and not expensive.
Uptown, with uptown prices, try Patsy's on the westsdie or Il Vagabondo on the eastside. They are the best!
Have a great time!

If he wants a true "Italian" experience. Stay FAR away form that tourist trap area!! Little Italy is a joke!
Go to Mario Batali's Lupa, in the village. On Thompson St.
Little Italy only has tunnel and bridge people. The food is horrendous and no flavor. Your better off going yo an authentic Italian restaurant.
If that does not float your fancy, go around the corner on 6th and go to DaSilvano.
Have fun!!

Dont go to little italy - go to Arthur Ave in the Bronx. Do to Domenicks for a real authentic meal. But beware - no menus, no privacy, no written bill. Be nice and they will not over charge you. Be a stickler and they will stick you! But the food is to DIE for! Bar upstairs for waiting - as they dont take reservations. For a more civilized meal, try Emilia's next door. After, try Enzo's next door (the other way) for dessert and drinks.

I would have to say Il Cortile, and they have side walk seating on warm days and a garden in the back which makes the experience even better!

Il Fornaio on Mulberry St. is inexpensive and the food is worth it. But for atmosphere and service Pelagrino's on Mulberry is great.
