Great Lent is the time of prayer and fasting, and is certainly the period of more intense spiritual struggle with the passions that torment us. As one of the chief means to struggle with passions, Confession is an important part of life for any Christian. By confessing our sins to God we purify our soul and He gives us strength to struggle further. He removes the burden of our sins through Confession, so we may be more prepared for the combat. Just like our bodies become more vigilant and energetic through fasting, so our soul becomes more prepared also. Although, any particular personal discipline (confession schedule) should be discuss privately with your spiritual father, you may want to do it as often as you feel it is necessary.
In the monasteries confession is made weekly, but for lay people, especially in our English-speaking countries it is recommended at least once per month.

Once, I asked my priest to perform Confession for all that I have done and he pretty much accepted what I told him as Confession. The Orthodox focus more on the Sacrament of Holy Unction which serves to cleanse and heal.
I can't speak for anyone else but Confession is something that one does in private or informally before a spiritual father. Just make an appointment and speak candidly to your priest and allow him to decide whether or not you need to confess.

Usually - once per month.
Lent: Usually twice. Once at the beginning (Shrove Tuesday or Ash Wednesday) and once near the end (at the Triduum: Maundy Thursday, Good Friday or Holy Saturday)

I confess my sins to my heavenly Father immediately. He has given me a promise "If we confess our sins, He is faithful and righteous to forgive our sins, and cleanse us from all unrighteousness." (I John 1:9)
I am so grateful the blood of Jesus has paid for all my sins, once and for all.
""but He (Jesus) having offered one sacrifice for sins for all time, sat down at the right hand of God, waiting from that time onward until His enemies be made a footstool for His feet. For by one offering He has perfected for all time those who are being sanctified." (Hebrews 9:12-14)
All you have to do is pray, believing, and take God at His word.
