Yes....it is sickening to the other people. Go to the washroom. I carry floss in my purse for that reason.

Yes. It's rude and disgusting. My father-in-law has a habit of doing this, whether I'm in their house or we're in a restaurant. It's rude and it's disgusting. Sometimes, he doesn't even bother with a toothpick, just his fingers. In a restaurant or at home with others, it's rude and gross and disrecpectful.

Well I personally think it is a bit rude to do so in the presence of others, the polite thing to do would be to excuse yourself and do it in the restroom.. either that or try to do so with your napkin as discretely as you possibly can.. or you can always try to see if you can use your tongue for it :) Just as long as no one knows you're doing it is fine in my opinion.. I mean what people won't know won't hurt them right? ;)

It depends on where you are eating and who you are with. If you sense that there is a huge piece of broccoli wedged in your tooth, say "may I be excused" and go to the bathroom.
