My previous employer that I worked with for only 6 weeks last year has not given me my tax information. I have tried contacting my supervisor and no response. I filled out a w-9 since it was sales and I was a sole proprietor. I plan on going to HR block next week to do my taxes. Am I able to give them how much I made(I know it from the check receipts) Plus my deductions from the job? Any help will be greatly appreciated

If you know the amount of income you earned in 2006, you can have your taxes prepared. You would file a Schedule C and be subject to self-employment tax on any net income.

Your employer can be subject to penalties. If any failure to file a correct information return is due to intentional disregard of the filing or correct information requirements, the penalty is at least $100 per information return.

You need to contact someone at the company you worked for other than your supervisor and ask them for the tax information you want/need.
You can tell your tax prep person how much you made and how much your deductions are and they can fill out a Schedule C or maybe even a CEZ with that information.

If you were considered self-employed and you made less than $600, they would not have to give you anything for tax reporting. The check receipts are your record for tax purposes.

If you were considered as self-employed, you can use your own records of income and expenses to file your return.

contact the IRS, they will get the info that you need if your employer still refuse's then they will get fined
