Yes matter does change form, and that change is constant. What is the only thing that is permanent? Impermanence.

Conservation of mass, conservation of energy. Yes it changes form, but it doesn't just go away.

you have a lot to learn about physics, forces, potential, and conservation of energy.

Science places the beginning of all things at the big bang, some 13.7 billion years ago. Asking how the big bang came to be or what was before that is something science has no answer to right now (and in the case of the before question, it is not clear if that question makes any sense).

Remember, it all happened a REALLY long time ago. Additionally, placing your God there, a 'god of the gaps in science' as it were, is a really bad idea. Chances are that science will someday close the gaps, and then where will your god be?

They cannot die because they are not alive.

Everything else you said is just meaningless gibberish. The forces were (in theory) unified in the early universe. If the universe collapses they will reunify. We do not know what might happen in an extremely cold, diffuse universe.
