Technically 6 but really 5
1- on December 21, 1997 he defeated Maurice Smith for the Heavyweight Title
2- on November 17, 2000 he defeated Kevin Randleman to regain the Heavyweight Title
3- on June 6, 2003 he defeated Chuck Liddell for the Interim Light Heavyweight Title
4- on September 26, 2003 he defeated Tito Ortiz for the Undisputed Light Heavywieght Title
5- On August 21, 2004 he defeated Vitor Belfort to regain the Light Heavyweight title
6- On March 3, 2007 he defeated Tim Sylvia to once again regain the Heavyweight Title
(the interim LHW title and Undisputed are the same title, tito was just being a pussy at the time and wouldn't fight Chuck, and in my opinion he never lost the title to Vitor as the fight ended after just one punch and was called because Vitor stuck his finger in Randy's eye and the doctor stopped it)
