What type of fistula does the child have? By this, do you mean a cleft palate? If so, the child should be seen by a cranio-facial specialist to discuss surgery to correct the problem or the use of an appliance, such as an obdurator to occlude the cleft. The child should be evaluated by a speech therapist who can recommend appropriate goals for therapy.

I would ask your therapist or the parents of the child for suggestions. One great place to get toys is the Lekotek program. Most Easter Seals locations are involved. The way is works is its a lending toy program. You borrow the toys for a set amount of time and return them for a different toy.
