I am not too sure, since I have no such advanced medical knowledge, but since I am studying entity-relationship diagrams and many similar things I might be able to give you some ideas.
first_name
last_name
patient_id
diagnosis
reference number
procedure_type
date_admitted ( when patient came in hospital)
Like I said, I am not sure of these, they are just ideas.
